People with bladder cancer usually attend with their GP with symptoms such as:
- Blood or blood clots in the urine
- Pain or burning sensation during urination
- Frequent urination
- Feeling the need to urinate many times throughout the night
- Feeling the need to urinate, but not being able to pass urine
- Lower back pain on one side of the body
This is often mistaken for a urinary tract infection (UTI) and treated with antibiotics whereas the patient in fact should be referred to a specialist urology surgeon for various tests. Perhaps even the patient was referred by the GP for urology review, but the urologist failed to take a proper history or organise investigations to include: full blood count, liver and renal function tests, bacteriological and cytological examination or an ultrasound. In some cases, the urologist may have ordered the appropriate tests but made an error in interpreting the investigations. In other cases, the urologist might fail to admit the patient for cystoscopy which is a procedure to look inside the bladder using a thin camera.
Treatment of bladder cancer depends on whether the tumour is superficial, invasive or metastatic. Superficial tumours are resected by diathermy which is deep heating at cystoscopy. This is accompanied by intravesical chemotherapy. Invasive bladder cancer is treated by radiation or surgery. Metastatic bladder cancer is treated with chemotherapy. Mistakes in diagnosing bladder cancer can be very serious particularly if the delay has caused more invasive and aggressive treatments than otherwise should be the case.
